ऐसे नि:शक्त व्यक्ति जिनके अभिभावक/पालकों की
वार्षिक आय रू. 96,000 से अधिक न हो,मध्यप्रदेश के निवासी हो ,विषय विशेषज्ञ के द्वारा
शल्य चिकित्सा की अनुशंसा की गयी हो , नि:शक्त व्यक्ति (समान अवसर ,अधिकार संरक्षण
और पूर्ण भागीदारी) अधिनियम 1995 की धारा -2 में यथा परिभाषित नि:शक्तता हो। पात्र
नि:शक्त व्यक्ति , अपने आवेदन पत्र संयुक्त संचालक/उप संचालक सामाजिक न्याय को जिला
स्तर पर प्रस्तुत करेंगे ।जिला अधिकारी द्वारा प्रत्येक पक्ष मे प्राप्त होने
वाले आवेदन पत्र के अभिप्रमाणन/अनुशंसा हेतु ,चिकित्सा महाविद्यालय/जिला चिकित्सालय
को प्रस्तुत किये जाते हैं। चिकित्सा विशेषज्ञ की अनुशंसा के आधार पर शल्य चिकित्सा
जिला कलेक्टर की स्वीकृति से की जाती है। आवश्यक दस्तावेज नि:शक्तता प्रमाण पत्र,निवास
प्रमाण पत्र,आय प्रमाण पत्र ,दो छाया चित्र।
For such a disabled person
whose parents / guardian's annual income is Rs. Should not be more than 96,000,
resident of Madhya Pradesh, surgery recommended by the subject specialist, ineffectiveness
as defined in Section 2 of the Disabled Persons (Equal Opportunities, Rights Protection
and Full Participation) Act 1995 . The eligible disabled person will submit his
application to the Joint Director / Deputy Director Social Justice at the district
level.
For the certification / recommendation of the application form, the medical
colleges / district hospitals are presented. Based on the recommendation of the
medical specialist, the surgery is done with the approval of the district collector.
